"I can write wonderful advance medical directives, perfectly suited to my
client's situation, but they are useless if nobody reads them! So once we have
created appropriate documents, the challenge is to make them accessible. I ask
clients to give copies to their decision makers and their doctors, to take a
copy to the hospital on "planned visits", and even to put a set of copies in the
